The NEW Outreach Card Request Process (as of October 1, 2011).
- We will now be providing the cards to you printed with your name, the students name and the end date of the course on them and we will also be charging $5 per card.
- When you are entering the names of your students it is imperative that you double check the spelling of each name.
- To ensure delivery to you we will be sending your cards by traceable UPS ground shipment. UPS does NOT deliver to PO Boxes; please ensure you are using a street address when requesting cards.
This is the PREFERRED METHOD for requesting cards. If you have less than 3 people or more than 40, you must submit a request for permission to our center. You will also need to use the email submission method available under the menus button ‘Outreach Training Request Forms’.
We have partnered with University of Texas – Arlington (UTA) to provide a web-based process for providing your 10 and 30 hour Outreach Training Cards.
- The following website is the new web-based Outreach Training Portal (OTP) location to request cards: http://outreachportal.com/
- In the brown horizontal banner click on ‘Portal User Guide’. This guide will walk you through how to request your 10 and 30 hour Construction and General Industry Outreach Training Cards.
- At the end of the request you will be required to pay $5 per card by credit card.
If the information on the report is OSHA compliant then you will receive an email when it is approved and we will complete processing the cards. The process for approval can take up to a week.
